Join a Conversation Group and/or Committee for the Virtual Long-Term Care at Home Pilot Project


What are we doing?

The Virtual Long-Term Care at Home team is leading a pilot project to support patients/clients, specifically those who are at higher-risk but who might have lower care needs (i.e. clients with early stages of Alzheimer's disease, risk of falls), to age at home for as long as possible. This project also supports caregivers experiencing burnout. By offering real-time and continuous remote monitoring (i.e., digital device worn as a necklace, bracelet, etc. that enables 2-way calling and an SOS button to a 24-7 call centre) to caregivers, the Virtual Long-Term Care at Home project aims to:

  • Delay or prevent the need for long-term care (LTC) admission
  • Connect clients to the appropriate care services in the community

Who should apply?

Caregivers (i.e., unpaid family members and friends) with lived experience. At this time, the pilot project is focused on tailoring services specifically to members living and/or accessing care in Vancouver.

*Please note*: If you're not currently a caregiver for someone who has lower care requirements (i.e., the client/patient you care for needs significant support or they might eventually be higher-risk but still have lower care needs) and still feel your experiences could be helpful for this pilot project, we'd love to hear from you as well. Please complete the application form.

How can you get involved?

Conversation Group: You are invited to participate in a virtual or in-person (Vancouver) conversation group to share about your experiences and needs when supporting a patient/client that is at higher-risk but requires lower care needs to continue to live safely from home. This engagement will gather your feedback on the new remote monitoring project and how it can be best implemented to support these patients/clients and and you, as their caregivers.

*Please note*: The pilot program will only serve Vancouver residents at this time but if you wish to contribute your experience from another urban context, we would like to hear from you.

Committee Meetings: We are inviting two advisors to participate in Virtual Long-Term Care at Home committee meetings. These meetings will happen at different times, but on average, you will spend a maximum of 1 day (7.5 hours) per month in total for all meeting requirements. The location of these meetings may vary from in-person at Dogwood Transitional Care Unit in Vancouver or virtually. Selected caregiver advisors will support on-going work throughout the pilot project process by offering the patient/caregiver perspective to workflows, communication materials and more.

What is the deadline to apply for these engagement opportunities?

  • If you are interested in participating in the conversation group and/or monthly committee meetings, please fill out the application form below!
  • Date: mid-March
  • Time of day: based on the times shared in the application form that work for the most people.
  • If you are a caregiver who may need help with respite or childcare to join the conversation group or participate in monthly committee meetings, or if you identify as a seldom-heard group—such as those facing discrimination or bias due to age, cultural background, location, sexual orientation, disability or income—please mention this in your application. Selected participants will receive an honorarium of $25 per hour.
