Home Support Client and Family Survey

Welcome to the Home Support Guideline survey.

The Home Support Guidelines are clinical (staff) documents that outline how home support is provided in client’s homes. The Home Support Guidelines explain what care tasks clinicians can (and cannot) authorize a CHW to perform, and, how they should be done.

  • The Home Support Guidelines are being updated to help ensure consistent care across the Health Authority, and to improve the safety of clients and CHWs.
  • Their purpose is to help clinicians make informed decisions on the provision of HS around the region.
  • The document can serve as an educational tool, providing insight into the rationale behind required actions, parameters of authorization and promote consistent application of the service.
  • One of our goals with the update is to be more transparent with clients about VCH’s Home Support clinical documents.

What are Guidelines?

Guidelines offer recommendation on the most appropriate way to achieve equitable, safe and consistent Home Support.

However, guidelines can also offer flexibility and, with strong clinician recommendation, can be adapted based on presenting circumstances. It is not possible to produce a document that fits all situations as clients are unique, staff come with different experience and resources do vary dependent on remote, rural or urban environments.

Your feedback is extremely valuable as you and your loved ones, are the ones receiving the care.

Survey instructions

Prior to completing the survey, please review the Power Point and Client Handbook attached.

Select option

2.

How many years have you/your family member received Home Support / worked with clients who receive Home Support?

3.

In 2021 a client facing document, called the HS handbook was created. It is found on the VCH public facing website and it is given to new clients at their first visit (practice from 2021 onwards).

It was designed for clients and their families to explain what the Home Support service is.

Have you seen this before receiving the survey?

4.

In contrast to the client HS Handbook, the new HS Guidelines, is a clinician focused document.

It's potentially 50+ pages (to be published online) and is written for clinician staff. It provides detailed guidance on the activities performed in the home. It outlines how the home support is provided and helps the clinician create the care plan that the CHWs see on their mobile phone.

Do you think most clients would also like access to the VCH HS Guidelines?

5.

What parts of the HS work / world would you like to find out more about?
