The Home Support Guidelines are clinical (staff) documents that outline how home support is provided in client’s homes. The Home Support Guidelines explain what care tasks clinicians can (and cannot) authorize a CHW to perform, and, how they should be done.
The Home Support Guidelines are being updated to help ensure consistent care across the Health Authority, and to improve the safety of clients and CHWs.
Their purpose is to help clinicians make informed decisions on the provision of HS around the region.
The document can serve as an educational tool, providing insight into the rationale behind required actions, parameters of authorization and promote consistent application of the service.
One of our goals with the update is to be more transparent with clients about VCH’s Home Support clinical documents.
What are Guidelines?
Guidelines offer recommendation on the most appropriate way to achieve equitable, safe and consistent Home Support.
However, guidelines can also offer flexibility and, with strong clinician recommendation, can be adapted based on presenting circumstances. It is not possible to produce a document that fits all situations as clients are unique, staff come with different experience and resources do vary dependent on remote, rural or urban environments.
Your feedback is extremely valuable as you and your loved ones, are the ones receiving the care.
