Thank you for your interest in being selected as a member of the VCH Community Accessibility Committee. Expression of interest quick facts:
- This form is designed to be completed in 10-15 minutes.
- All responses are voluntary with two exceptions: we will need your contact information so that you can be reached about the committee and for you to share your relationship to the disability community.
- You can skip any questions you might not be comfortable answering.
- We include questions about personal and demographic information as a part of the requirements outlined in the BC Accessibility Act and to help us include a fuller representation of lived experience.*
- Your involvement on this Committee will not be disclosed unless you agree to the information being shared.
- You may withdraw from this expression of interest at any time.
- All of your responses are private and confidential. Responses to these questions are stored on a password-protected site and only available to members of the Community Engagement Team and Equity Team working with the Committee. - All personal information is collected under section 26(c) of the Freedom of Information and Protection of Privacy Act BC and will only be used for the purposes outlined above. The personal information collected will be securely stored within Canada and in accordance with the BC Ministry of Health.
