Focus Group or Survey - "My Medications May Have Changed" leaflet

Consultation has concluded. A focus group was organized in order to gather feedback on the leaflet.

What are we doing?

VCH wants to help patients better understand their medication plan after they leave hospital, as some patients may not adjust their medications after their hospital stay. This might happen when a patient does not remember that they have new medications to take or that they need to reduce the dosage or stop the medications that they are taking after discharge. (Sometimes leading to preventable harm.) While there is a process in place to give patients a prescription at hospital discharge, we find that many patients are not aware of their medication changes. It seems difficult for health care providers in hospital to have a careful conversation with the patient to ensure they understand their medication changes.

To address this, VCH designed this leaflet for the patient to take with them to their community pharmacy (when they fill their prescription). The leaflet directs patients to ask the community pharmacist four questions that will help ensure that if their medications were changed that the patient knows about these changes and knows exactly what they need to do afterwards.

How YOU can get INVOLVED

We would like your feedback on the leaflet:
