What are we doing?

The Lions Gate Hospital is looking for feedback for how to make patient discharges to their homes better after receiving care at the hospital.

When a patient goes home after receiving care at a hospital, this process is known as being "discharged". The discharge process can sometimes be complicated, and getting feedback to make it better means a better experience for patients and their families.


How YOU can get INVOLVED

Register your name in the form below and we will arrange for an individual and/or group interview in order to take your feedback.

Question may include:

  • What does a good patient discharge look like?
  • What is the impact on the patient when a discharge doesn't go smoothly and what pieces are necessary to ensure patients have a less stressful transition home?

At this time, only five registrants are looking to be interviewed. All others who apply beyond the five selected for an interview will be thanked for their interest and encouraged to apply to future feedback opportunities.

Who should register?

Ideally you and/or a family member or someone else you are close to had received care at the Lions Gate Hospital and can provide feedback on the experience you or they had when discharged from the hospital.

interviews to gather your feedback will take place by telephone and/or Zoom. Interviews are being scheduled between August 30th and September 10th. Interviews are being extended until September 30th.
