Join the Regional Home Health and Home Support and/or the Personalized Support and Stabilization Plus Community Advisory Committee

What are we doing?

Vancouver Coastal Health (VCH) is creating two advisory committees because we recognize that clients, family members, caregivers and staff are a valuable and critical resource.

  • The Home Health and Home Support programs assist clients to return and remain home after acute (i.e. hospital) stays by offering quality health care and support to promote independence, health and wellness through the delivery of person and family-centered care.
  • The Personalized Support and Stabilization Plus (PSS+) program is designed to meet the needs of the frail elderly during their transition from acute services to the community, through the provision of a high quality, safe, integrated, comprehensive, transitional care and stabilization interventions.

Over the next years, the demand for these services at VCH is projected to increase significantly with an additional increase in complexity of clients. VCH leaders and staff want to improve care coordination, care accountability, and meaningful partnerships with primary care to ensure the highest quality of care to its clients.

How YOU can get INVOLVED

VCH is looking for members who join these advisory committees to provide their input, perspectives and experiences on the policies, processes, programs, projects, priorities and transformation of the Home Health and Home Support as well as PSS+ programs. Past or present experience with these services is welcome! Please read the Terms of Reference for each advisory committee, located at the top of this page.

Who should apply?

The regional Home Health and Home Support teams are looking for ten passionate community partners, clients, family members and/or caregivers from Vancouver Coastal Health (VCH) to join the Home Health and Home Support Advisory Committee.

The regional Personalized Support and Stabilization Plus (PSS+) team is looking for two passionate community partners, clients, family members and/or caregivers.

This invitation is extended to anyone who is living or working in the Vancouver Coastal Health region with the following experience:

  • Lived experience
  • Family of a person with lived experience
  • Non-paid caregiver (i.e. friend or neighbour of someone with lived experience)
  • Representative from a non-profit organization serving clients/family members with lived or living experience

A representation of diverse perspectives is an important consideration for this committee. This includes but is not limited to experiences with: geography (urban, rural, remote), culture, language, age, gender identity and disability. If you are a current client or caregiver for someone in these VCH programs, any experiences or opinions shared will remain confidential and will not impact the care you are being provided.

What is the time commitment?

  • Meetings are once a month, 1.5 hours with some reading and emails between meetings
  • Membership is for one year
  • Location is on Zoom
