The Virtual Long-Term Care at Home team is leading a pilot project to support patients/clients, specifically those who are at higher-risk but who might have lower care needs (i.e. clients with early stages of Alzheimer&#39;s disease, risk of falls), to age at home for as long as possible. This project also supports caregivers experiencing burnout. By offering real-time and continuous remote monitoring (i.e., digital device worn as a necklace, bracelet, etc. that enables 2-way calling and an SOS button to a 24-7 call centre) to caregivers, the Virtual Long-Term Care at Home project aims to: <ul><li>Delay or prevent the need for long-term care (LTC) admission </li><li>Connect clients to the appropriate care services in the community</li></ul>Who should apply?Caregivers (i.e., unpaid family members and friends) with lived experience. At this time, the pilot project is focused on tailoring services specifically to members living and/or accessing care in Vancouver.*Please note*: If you&#39;re not currently a caregiver for someone who has lower care requirements (i.e., the client/patient you care for needs significant support or they might eventually be higher-risk but still have lower care needs) and still feel your experiences could be helpful for this pilot project, we&#39;d love to hear from you as well. Please complete the application form. How can you get involved? Conversation Group: You are invited to participate in a virtual or in-person (Vancouver) conversation group to share about your experiences and needs when supporting a patient/client that is at higher-risk but requires lower care needs to continue to live safely from home. This engagement will gather your feedback on the new remote monitoring project and how it can be best implemented to support these patients/clients and and you, as their caregivers. *Please note*: The pilot program will only serve Vancouver residents at this time but if you wish to contribute your experience from another urban context, we would like to hear from you. Committee Meetings: We are inviting two advisors to participate in Virtual Long-Term Care at Home committee meetings. These meetings will happen at different times, but on average, you will spend a maximum of 1 day (7.5 hours) per month in total for all meeting requirements. Join a Conversation Group and/or Committee for the Virtual Long-Term Care at Home Pilot Project

Richmond Lions Manor-Fentiman is a new campus of care with a total of 144 long-term care beds and a 14 bed hospice unit. The home also includes an adult day program and overnight respite and a 37-space child care centre.It will be located at the original site of Richmond Lions Manor at 11771 Fentiman Place, which was demolished after serving the community for several decades. Residents are being cared for at the temporary site located at Richmond Lions Manor Bridgeport, and will move to the new campus of care when it opens.The capital cost of the project is estimated to be $177.7 million and will be fully funded by the Province of British Columbia.
